What the work involves
Medical AI training means using your clinical judgement on a model's answers, not treating patients. Mercor's medical listings describe two main tasks: writing a case from your own practice, with the presentation, the workup and the constraints, and grading model output on clinical reasoning. Handshake AI's medicine listings describe writing domain prompts and rubrics and evaluating answers on diagnosis, treatment planning, surgical risk and psychiatric care.
Where nurses and doctors can apply
- Mercor, medical expert pool. A standing listing for clinicians and health professionals, which Mercor says is not a specific job opening. Past listings across the field have posted $60 to $180 an hour, set per project. When we checked on 8 November 2026 this listing had stopped taking applications and pointed to a replacement network covering the same expertise. See the listing. An older Mercor registered nurses opening paid $60 to $110 an hour and is closed.
- Mercor, Medical Expert (Spain). A specific opening, taking applications on 8 November 2026: $120 an hour, about 21 hours a week, for a pilot running 23 October to 19 November 2026, for physicians licensed in Spain who are fluent in Spanish. See the listing.
- DataAnnotation, professional tier. Its FAQ lists medicine, law and finance work for licensed professionals starting at $40 to $125+ an hour. Read the FAQ.
- Appen (CrowdGen). Its home page lists medical clinicians at up to $450 an hour and healthcare administrators at up to $90 an hour. These are ceilings, not typical rates. See CrowdGen.
- Handshake AI. US listings for physicians ($175 to $200 an hour) and for medicine experts including nurses, physician assistants and pharmacists ($110 to $140 an hour) were closed when we checked, but show what this work has paid.
What you need to apply
Expect to show a clinical credential and professional experience. Mercor asks for "a clinical credential and professional experience in a health profession", and its Spain role needs a Spanish medical licence. Handshake's physician roles asked for an active or previously active licence in good standing and direct patient care experience. DataAnnotation's professional tier asks for licensed credentials. Expect an assessment or AI interview as well.
Mercor's listings say it cannot support US H-1B or STEM OPT candidates.
What it pays
On published figures, the ranges above run from $40 an hour (DataAnnotation's starting point) to $180 an hour (top of past listings in Mercor's medical pool), with higher one-off listings. CBS News reported a Mercor psychiatry listing paying up to $350 an hour and an average of $105 an hour across Mercor's AI training jobs. Pay is set per project, so check each listing.
Things to know before you start
- This is independent contractor work. Mercor and Handshake both say so on their listings, and there are no benefits or guaranteed hours.
- Projects start and stop with little notice. As PYMNTS put it in October 2026, "the work does not always last".
- Licence checks are country specific, so a licence from one country may not qualify for a role in another.
- You are grading and writing training material, not giving medical advice to patients.

Questions
Do I need an active licence?
It depends on the role. Mercor's Spain role needs a current Spanish licence, while Handshake's US physician roles accepted an active or previously active licence in good standing. Mercor's general medical pool asks for a clinical credential and experience.
Can nurses do AI training work?
Yes. Mercor has run openings for registered nurses (the one we found was closed on 8 November 2026), and Handshake's medicine role was open to nurses, physician assistants and pharmacists.
How much do doctors earn training AI?
Published listings we found ranged from $60 to $200 an hour, with a reported Mercor psychiatry listing at up to $350 an hour. Rates depend on the project and your specialty.
